Requested new patient paperwork via email (extensive) and completed prior to appointment. This is a must!
In order to complete all these forms legibly it will take an average of 30+ minutes. Include a copy of your living will and medical POA. Best to do this at home and you will only need to provide your insurance card(s), drivers license, and stack of completed paperwork when you arrive.

The doctors office was really good. I called him. Let him know I was running late. They said no problem soon as I walked through the door, they had me go to the back and started right away A no nonsense Doctors Office that's what you look for they're one and 1 million That Fredericksburg office now the Fairfax office that's packed And you better not show up late because they just might send you home With that said they play around either they get you gun in the back They get you going no hesitation they know their plan they stick to it and they get a job on these ladies work hard and should be appreciated just as much
