Did not adequately monitor or treat heart issues in a pt with a family of heart issue. Saw pt at the beginning of month due to heart palpitations and concerns. Ordered no follow up testing. Patient died of a heart attack 3 weeks later at the same age his father had his first major heart attack. Much could have been done. Instead, we've all lost a dear loved one.
