My time spent with Dr Liebo is always spent wisely. He listens to my concerns along with telling how my test and blood work came back for me. We also discuss my future plans for well being. My name is David Voo. My Patient Randomination number is 23527. I joined ION-682884 group for Cardic Amyloidosis Clinic since Jan 8, 2021. Dr. Max Liebo, MD is the group Leader representing Loyola. His research nurses name is Carol Kartje, RN, BSN. My opinion and medical evaluation of them are very good. I do have one complaint however, and it is directed towards the Accounts Payable Department. According to ION contract that both Dr. Liebo signed in the beginning of 2021, on page 16 of the Contract, quote, "For your time and effort, you will be compensaed $50.00 for each required in-clinic study visit that you complete". To date, I was told to travel to Layola Maywood15 times. Each trip was requested by Dr. Lieba and Nurse Carol. The dates in 2021 were: 1/8; 1/25; 1/29; 4/1; 4/8; 4/15; 4/29; 5/27; j/24; 7/2; 7/9; 7/26; 8/23; 9/16; 10/15. Each visit was requested and REQUIRED by Dr. Liebo and Nurse Carol. Each trip consumed about 3 hours because I live 37 miles away from Maywood. 15 Required in-clinic study x $50 each = $750. I have written a few months ago to Accounts Payable with descriptions and copies of 4 exhibits. I have received no response. Before that, I have submittdW-9, and that was successful. I have also able to get through on the phone to Patti Spears, Accounts Payable Represetative, she promised to help and return my call. The call never came. Can you help? Sincerely, David Voo
