Worst experience every, had to pay $10 for a physical form to get filled out, mind you this is my primary Dr. I have never in my life herd of a $10 fee, its been almost a month and my employer still hasn't receive it, even after numerous phone calls, I keep getting the run around. They even told me to call in a half an hour I called the office was closed. I still am waiting for the paperwork, I am not getting paid from my employer because of it I just give up I'm going to pay for another physical to get my papers filled out. At this point I am looking for a new primary doctor. THEY DO NOT EVEN DESERVE ONE This was the only way I was able to post it!
